Richard Calapaz (EF Education-Easy Post) and Enric Mas (Movistar) crashed on the descent of the Alto del Vivero in the finale of stage 1, ruining their overall challenge in the Tour de France.
The two went down in the same crash with 23km to go in the peloton, which lost pace on the run-in to Bilbao.
Mas was forced to retire with what appeared to be a shoulder injury, while Calapaz lay on the shoulder for over five minutes. Carapas, the Ecuadorian champion, suffered cuts on both knees and looked distressed as he tried to pedal to the finish, many minutes behind the peloton.
According to Team Movistar, Mas had severe pain in his right shoulder and will undergo tests at the hospital.
Kalapas finished more than 15 minutes behind stage winner Adam Yates and received sympathetic cheers from the roadside crowd.
"He looked in a lot of pain. It's a blow. It can happen to anyone. There was no crash today, so I think that had a big impact," EF Education - Easy Post director Tom Southam told Cycling News.
Kalapas, who moved from Ineos in the off-season, started this Tour as EF's overall leader, but his time loss and injury will force the team to modify its plans for the remaining races.
Nielson Powles led the race over the Vivero to take his first mountains jersey of the event.
Rigoberto UrĂ¡n, who finished on the podium at the 2017 Tour, lost more than five minutes on Saturday's stage; EF's Tour team also includes Alberto Bettiol and Magnus Korto, who will be looking for stage wins.
"We need to think about the next step," Southam said. 'But if you look at the teams, there are a lot of players who could step up. There are many other options. We didn't come here with seven guys for Richard and him."
